Minecraft where one block will instantly kill you and delete your save. You may never encounter it but it exists in the world. What sadistic feature could you put in another game?

 Super Mario Bros. One coin in the entire game is an instant game-over. It can spawn at any part of any level each time you start a new game. But also you need to collect coins to keep the timer above zero and each level starts at 30 seconds.

A Zelda game where every time you get an item or rupee in the overworld you get a message screen detailing that item along with an animation.

And you can't skip it for 4.5 seconds... And it prompts you "are you sure you want to close this window?"
